PRICING

Elevate Mofunctional Therapy offers a Fee-for-Service model. This means that we are out of network for all insurance providers and do not bill insurance directly. Payment for services is provided directly by patients to Elevate Myofunctional Therapy. Once payment is receieved, patients can request a monthly superbill that they can then submit to their insurance to seek possible reimbursement. Elevate Myofunctional Therapy makes no guarantee of reimbursement. 

 

To receive services, patients must have a payment card on file. Payments will be made electronically at the time of services. These payments can be made with debit/credit cards or HSA/FSA cards.
